Has anyone developed a value stream map of the discharge process, would you mind sharing. I get the gyst but am struggling with a few things and thats why i am looking to see how others depict things like electronic medication reconciliation etc. I check messages on this forum off and on and can be reached at nsachdev@cfhalliance.org.
