Dear members,
I am currently trying to figure out how to optimize a Pharmacy process through Lean. Besides 5S and inventory analysis , i see there is a lot of waste in the mixing processes. Many liquids and medicines all over the place and a lot of documentation. Any advise on optimizing this type of process? Any past experience I can learn from?

Hey Andrea,
As part of the 5S program; concepts of visual management, standard work and PDCA are usually introduced. Have you looked into elobrating on those concepts?
In a hospital pharmacy that I've worked with, we've introduced the idea of a performance board, huddles and kaizen newspapers to engage the staff in taking action to resolve issues of 'what's not working' as part of the 5S Kaizen we've conducted.
Just some thoughts...

Thank you Rick, I want to do something else besides 5S. There are ways to change bins to fit more things, make it more visual, etc. I see a big challenge in the way they have been thinking, and in the Mixing process they always have "reserve inventory, just incase..." and I see there might be a problem with this. For the oncology Mixing there is one cart full of specialized medicine jus for "just in case...". I´ve also noticed they tend to work in batches. This is completely against lean, but in this case where they go into the sterile room, they go in the morning and prepare everything for the hospitalized patients. Maybe work in shifts and minimize those batches? See a lot of labeling with patient´s name, id, etc. Pharmacists print and past... Many opportunities, I just want to look at maybe different projects to learn best practices. Thank you for your response!

Hi Andrea,
From reading your comments, it sounds like you need a series of kaizens to;
- change the traditional thinking of frontline staff
- expose the 8 waste for the staff to see the broken processes & cost
- lead the staff in developing and sustaning process changes that they feel it'll add value
May I recommend you start with a Value Stream Mapping(VSM) session; a 2-3 day planning session to identify and prioritize the opportunities. We partly use this event to get staff and management onside and agree on the issues and opportunities.
From the VSM, the opportunities could then be categorize as just-do-its; kaizens and projects.

Hello Ricardo,
Here is an example of a daily performance board that the chemotherapy pharmacy used to track production. They discussed this at their daily huddle every day at 3pm.
At 11am they do a check of the # of 8:30, 9:00 and 10:00 patient orders they still have to prepare for and similar at 3pm. 'Green' indicated that production is ok and patient are getting their drugs administrated within 1hr of their appointment time. The information is relatively simple on this board as this was the pharmacy's first introduction to measure of daily production. I believe now, they're started to graph the data point and trend the results.
