		<description><![CDATA[ Healthcare has just recently become somewhat willing to allow ideas from the outside world seep in.  I'm looking for creative examples of these.  The checklist concept from the airlines industry has been well documented on how that was incorporated into the OR.....there are other examples from Disney and the Ritz Carlton, but am looking for others that may not appear as "big" examples or are not the usual benchmarks.  And they don't necessarily have to be in the clinical environment.  Thoughts?  Thanx in advance. ]]></description>

		<description><![CDATA[ B]Current Situation:</b><br />A chemical sample comes to a lab (in a small container). The sample then gets between 4 and 12 tests depending on it's requirements. For this to happen, a sample gets handed off from the initial Receptionist to about 4 different Techs who conduct the tests, then back to the Receptionist.<br /><br /><b>Problem:</b><br />There are occasions when a sample and it's paper work get mixed up with other samples and paper work (i.e., sample A ends up with sample B's paperwork). As you can imagine this creates alot of extra work correcting this defect. In addtion, is gives the Lab a less than desireble reputation with their internal customers who provided the sample.<br /><br />We have a kaizen team (Lab Techs and their Superintendent, Lean Coach) that will be walking the process and mapping it out in an effort to identify where these "mixed samples" could be occuring in the process and why (root cause) they are occuring. From there we will work on countermeasures, then check to see if they work (PDCA).<br /><br />The input I was hoping to recieve from this forum is - <br />a) has anyone found actual countermeasures to prevent a problem like this, and what are they?<br />b) any other thoughts, ideas, suggestions to help solve this problem?<br /><br />Thank you all and looking forward to reading your responses. ]]></description>

		<description><![CDATA[ More a Six Sigma topic really, but I do use Six Sigma tools as part of my lean approach. What I'm wondering is if anyone has any good case studies demonstrating Simple Regression Analysis in healthcare settings. If so please let me know. I have a few basic examples but am interested in learning more about how this concept and basic correlation is used in healthcare.<br /><br />I'm a firm believer in using SPC as part of Lean but unfortunately many in healthcare are not so willing to consider the value of these concepts.<br /><br />Thanks<br />Brian ]]></description>

		<description><![CDATA[ Does anyone have experience with audits from the FDA and lean practices?  Specifically with standard work and the detail of standard work compared with SOP's, work intructions, and etc...Our SOP's are vague on purpose (we don't want to divulge our IP to clients) and we want to implement standard work that is more detailed but are concered that we will have an unordinate amount of deviations introduced into our deviation process. The problem is that we are not developing as i´ve had hoped. I understand it´s hard to give advise when you dont know our status. 		<description><![CDATA[ I am putting together some training on how to create value stream maps, more specifically I plan to teach the class how to create swim lane charts.  As a part of this training I am going to do an interactive current state/future state mapping.  I have an example of a business process that I have used in the past, and also an example of a manufacturing process, but no healthcare example.
