Just found this today. I only added a couple headers under Monthly:
First Column = Week
Second Column = Day
Third Column = Task

Here are a couple of examples we use at our facility, and an audit checklist for auditing LSW. We print on 5.5" x 8.5" paper and put them in our "Arc Binders" from Staples.

Hello Kelly,

Thank you for the examples, can you give some extra feedback about the different colloured fields on the supervisor sheet? Daily,weekly and monthly tasks is clear, for the other items i would like to have some extra info: yellow, blue, pink and orange colloured fields.

i am looking for a template that would be able to give %ge adherence to LSW on a daily basis.

this will form a good start and i will see how i can build on it
